Potential Occupational Safety or Health Hazard Preventive Safe Work Conditions, Practices and Equipment
General work area Existence of unsafe conditions of which management may not be aware Report all unsafe conditions or practices to supervisor.

Report all accidents, injuries and illnesses to supervisor.
  Hazards associated with fire & attempts to control fire Activate the nearest fire pull station.

Call the Fire Department at 9-1-1.

Notify the main desk.

Employees must stop work and evacuate to designated areas.

Know emergency action plan and fire-evacuation plan rules.

No one should extinguish fires unless trained to do so.
  Misconduct & substance abuse causing carelessness & unsafe operations Unprofessional conduct is prohibited.

Employee unauthorized use of intoxicants is forbidden during working hours and on the premise.

Use of illegal drugs is prohibited.

Be aware that prescribed medicines or over-the-counter medicines can have an effect upon your ability to work, drive or operate machinery.
  Fire hazard & tobacco smoke exposure No smoking allowed in building.
  Fire extinguishers blocked Fire extinguishers and emergency exit doors must be kept clear.
  Means of egress blocked; door blocked Employees must keep means of egress free of obstruction and unlocked during business hours.
  Aisles blocked with storage Aisles and hallways must be kept clear.
  Existence of unsafe conditions due to curled rug or broken mat or surfaces which may cause a fall Secure all floor mats so they lie flat.

Secure carpet to prevent curling.

Notify management of unsafe surfaces.
  Trips and falls Keep work areas clear of debris,
and electrical cords; do not use floor for excess storage.

Be cautious; walk, don't run.

Clean up spills immediately.
Hold handrails on stairs.

Secure electrical cords and keep them out of walkways.

Use a ladder; do not stand on chairs, desks or tables.
  Being struck by opening door Open doors slowly.
  Cuts and/or lacerations Use care in handling materials and tools
  File cabinets tipping over Only one drawer should be open at a time.

Load cabinets from the bottom first.
  Personal injury from equipment Use personal protective equipment
as required for the job.

Do not operate equipment unless trained to do so.
  Electrical hazards Pull electrical cords by the plug.

Use properly grounded plugs.

Adapters should not be used.
Lifting and carrying Back and extremity strains Use proper lifting techniques.

Use handcarts and/or lifts for heavy loads.

Ask for help when necessary.
Break areas/lunch rooms Burns from liquids or steam Exercise caution when pouring hot liquids and using appliances, such as toaster ovens or microwaves.
Break rooms/bathrooms Slipping on liquid on floor Wipe liquid off floor as soon as it is spilled.

Mark spills to warn others.

Report all spills.
Material/chemical Toxic exposure Proper personal equipment (gloves, masks, apron, respirators, goggles and clothing) must be worn.

Wash hands well after using any chemicals.

Use caution when spraying aerosol products.

Notify supervisor of any material handling exposure.
  Spills If substance is unknown, do not clean it up; mark it and report it at once.

If not trained for clean up, do not attempt.
Filing Pinching fingers and body parts Use care in closing drawers, ensure all hands and fingers are clear.
Electrical equipment Electrical shock No extension cords over 6 feet long.

No adapters or gang plugs should be used.
Driving Motor vehicle accident
and/or vehicle accidents resulting in personal injury
Follow driver safety codes.

Practice defensive driving skills.

Avoid rushing; allow adequate time.

Wear seatbelts and shoulder harness at all times.

Observe posted speed limits and appropriate speed for road conditions.

Practice defensive driving.

Park in legal space only and do not obstruct traffic.

Driving under the influence of alcohol, drugs or the use of other intoxicants is prohibited.

Ask doctor or pharmacist if prescribed or over-the-counter medicines have an effect upon ability to drive.
